The AMTA forms have been created to comply with government regulations that telecommunication carriers need to comply with. By law information on the AMTA form must be obtained during the point of purchase and posted to the address mentioned on the form. It is part of the commercial agreement between both you as the retailer and us on behalf of Optus, as such failure to comply with the regulations is failure to comply with the agreement and therefore the agreement can be terminated.
